you in the realm of Star Trek fan fiction.

The following is how I would like you to prepare your contact
information:

I.  Contact Sheet
	This section collects general information pertaining to the
applicant's offered services.

Contact Name:
	(This may be your real name, internet identity, pen name, etc.)
E-mail Address(es):
Rating Floor:
	(From G to NC-17, the lowest acceptable content rating you will
	 beta read)
Rating Ceiling:
	(From G to NC-17, the highest acceptable content rating you
	 will beta read)
Specialties:
	(Proof reading, Sentence Structure, Grammar, Characterization,
	 Plot, Setting, Writing Style, General Trek Knowledge.  Select
	 all that apply.)

II.  Series Information
	This section specifies the applicant's criteria for a given
	series.  INFORMATION SHOULD BE SPECIFIED SEPARATELY FOR EACH
	SERIES OF INTEREST.  For example, if you beta both DS9 and
	Voyager stories, complete the section once for DS9, and once
	again for Voyager.

	(TOS, TNG, DS9, VOY, ENT, Other)
Genre:
	(Romance, parody/humour, angst, hurt/comfort, erotica,
	 character death, alternate universe (AU), crossovers, episode
	 additions (codas, missing scenes, etc.), poetry and general
	 adventure.  Select all that apply.)
Erotica Genre (if applicable):
	(Slash, Het, Mult, B/D, S&M, BDSM, D/s, NonCons, Rape.  Select
	 all that apply.)
	If my sources are still accurate, slash (in ASC) refers to
	same-sex pairings, while het denotes m/f pairings.
Pairings:
	(List relevant pairing codes, or All)  I don't know if it makes
	 any sense to make a distinction between erotica and general/
	 romance pairings.  I figure if you read certain types of
	 erotica, you're probably inclined to read no-sex stories with
	 the same pairings also.
Preferred Pairings/Characters: (If applicable)
	For applicants who list many codes but may have a fondness for
	one or two in particular.
